Appointments

Appointments

You may schedule an appointment by calling our office. If you need to reschedule an appointment or if you will be late for your appointment, we ask that you call as soon as possible so we can call our patients on the waiting list. Please bring your insurance card with you to the office during the time of your appointment if you are a new patient or have just received new insurance. If you are a new patient, you may print the New Patient Information Form that is located under the New Patients tab and fill it out prior to your visit to expedite the waiting time in the office.

After Hours

Please call the office and our answering service will assist you.

Billing

If we are contracted with your insurance carrier, our office will file your insurance claims directly to your health insurance carrier. If you should happen to change your insurance during the course of treatment, please communicate this promptly to our office to avoid any unnecessary bills to you. All copays are due at the time of service. Our office accepts cash, personal checks, Visa, MasterCard, and Discover. If you have any questions regarding your account, please call our billing personnel at 720 291 - 5201

Disability Forms

If you have a disability form that needs to be completed by our office staff, please remember that it takes five (5) business days for processing. There is a minimum charge of $20.00 for completing the forms. The forms can be mailed, faxed, or picked up upon receipt of payment.

X-Ray Request

Physicians are required to maintain medical records including X-Rays for 7 years. Therefore, X-Rays are a part of your permanent medical record. If you would like to receive a copy of X-Rays, you can purchase them for $10.00 per film (sheet). Please allow a minimum of 5 business days for processing you request.

Medical Record Request

All medical records requested must be authorized by our patient and made in writing by a formal written release. A copy of the Authorization to Release Records is available to you in our office. All record requests will be processed within 30 days of the written request. A processing fee of $14.00 for the first 10 pages, then .50 each page thereafter up to 40 pages and .33 each page after 40 pages plus postage will apply unless it is requested by a licensed physician by any state for the purpose of emergency or acute care.

Test Scheduling

If you were referred for a test (MRI, CT, Nerve Conduction Study, etc.) our office staff will make the appointment for you and contact you once scheduled. On some occasions, test scheduling cannot be completed until the office notes have been transcribed and sent to your insurance company. As a result this may delay your test scheduling. Contact us if you have not been notified regarding the scheduling of your test.

Test Results

Schedule a return office appointment within 48 hours of your test completion. The doctor will discuss test results with you at this time.

Prescription Refills

To facilitate the processing of your prescription refills, please call your pharmacy for any refills requests. The pharmacy will call our office with any questions regarding your refill.

If you are requesting a new medication or a different medication from what you are taking, please call our office directly and speak with our staff who will ask the physician. To process any new prescriptions efficiently, please have your pharmacy number available.

If you are taking a pain medication, you must be seen by our physician a least every 3 months in order to obtain a refill.

If you are taking an anti-inflammatory medication, you must be seen by our physician at least every 6 months in order to obtain a refill.

Our office will attempt to process all prescription request within 24 hours unless Prior Authorization from your insurance company is needed. Refill requests will not be processed after office hours or over the weekend.
